Hospice Medical Director
Is the physician who coordinates the team to create an individual plan of care tailored to each individual patient’s needs based on their symptoms and wishes. Our Medical Director is unique in that she is available 24 hr/day.
Is the nurse who visits you regularly to assess, organize, and implement your plan of care based on your individual wishes. Hospice nurses are specially trained to treat symptoms and increase comfort. The are also available to you 24/7 in case of a crisis.
Helps assess the patient’s and family’s psychosocial and financial needs to make sure those are met. The social worker can also provide direct counseling and arrange referrals to community agencies as necessary.
In treating the whole person-body, mind and spirit-the chaplain (non-denominational) assesses the patient’s and family’s spiritual needs. They are also helpful in providing bereavement counseling for a minimum of 13 months after the death of a loved one.
Certified Nursing Assistant (CNA)
Assists the patient and family with personal care needs such as bathing, dressing, feeding.
After attending volunteer training at the hospice, hospice volunteers can give caregivers a rest, help with errands, or provide companionship.
